Smart Referral Network

A Smart Referral Network (SRN) is an innovative, locally administered, web-based platform designed to facilitate and track referrals between service providers, ensuring that individuals and families in need receive timely and coordinated support. SRN enables agencies to share client-level referral data securely, generate an unduplicated count of clients served, identify service gaps, and measure the effectiveness of community programs.

SRN was developed in response to the growing need for integrated service delivery and data-driven coordination among agencies. Many funders and stakeholders now recognize the value of closed-loop referral systems and encourage their adoption to improve outcomes and accountability. In our region, Kings United Way serves as the lead administrator of the SRN, helping us better understand community needs and strengthen collaboration among providers.

Participating in SRN offers significant strategic advantages for service organizations. The SRN platform supports multi-level data sharing, allowing agencies to track referrals from initiation to completion while maintaining strict controls over sensitive information. The system prevents service duplication, enhances provider communication, and streamlines the referral process. SRN also offers a comprehensive suite of reporting tools and customizable workflows to meet the unique needs of each agency. As a web-based solution, SRN can be accessed securely from any device with internet access.

Providers who use the Smart Referral Network are better equipped to demonstrate effective service coordination and are often more competitive for funding opportunities. While participation is not mandatory for all programs, it is strongly encouraged to ensure a more complete picture of community needs and to foster a culture of collaboration and continuous improvement.
